between 1 and 2 o'clock

this is a way of pinpointing the area. It means if you were looking at a clock, where 1 and 2 o'clock are on the clock's face; in other words, slightly clockwise from straight up.

-If you can imagine the patient in SSL - lithotomy position - (as if giving birth) and the physician examining the *area*, 1-2 Uhr would be on the right for the physician, but on the left as far as the patient is concerned.

1 to 2 o'clock

This has to do with the location or position of the Druckschmerz. So if it's at 1 to 2 o'clock, it's roughly in the top right-hand area.
