Product Description
Product Y is indicated to delay disability in patients with mild dementia of the Alzheimer ’s type.
Product Y is the first and only selective amyloid lowering agent (SALA)—a unique new class of drugs shown to reduce amyloid beta (Aβ42), a toxic peptide that kills neurons and initiates the formation of senile plaques in preclinical studies. It is believed to play a central role in initiating and perpetuating the clinical signs of Alzheimer’s disease (AD). Product Y may be used alone or in combination with other AD treatments, such as acetylcholinesterase inhibitors, NMDA receptor antagonists, or both.
Efficacy
An 18-month, multicenter, double-blind, placebo-controlled trial was conducted to establish the efficacy and safety of Product Y. The trial included more than 1600 subjects (MMSE 20 to 26) randomized (1:1) to active drug (Product Y) or placebo, in addition to any stable AD treatment (acetylcholinesterase inhibitor or memantine) they were taking upon entering the trial (see chart below). Efficacy data therefore reflect the benefit of Product Y above the current standard of care.
The study shows a statistically significant benefit for patients on Product Y versus placebo in cognition and activities of daily living at 18 months as measured by the ADAS-cog and ADCS-ADL. Produit Y est le premier et le seul agent réducteur sélectif des amyloïdes (SALA, selective amyloid lowering agent), c’est une nouvelle classe médicamenteuse unique. Des études précliniques ont montré que cette nouvelle classe diminue la bêta-amyloïde (Aβ 42), un peptide toxique qui tue les neurones et initie la formation de plaques séniles. Purpose
The main objective of this study is to compare a Dual Antithrombotic Therapy (DAT) regimen of 110mg dabigatran etexilate b.i.d. plus clopidogrel or ticagrelor (110mg DE DAT) and 150mg dabigatran etexilate b.i.d. plus clopidogrel or ticagrelor (150mg DE-DAT) with a Triple Antithrombotic Therapy (TAT) combination of warfarin plus clopidogrel or ticagrelor plus ASA
Translation - French Évaluation de la double thérapie avec le dabigatran par rapport à la triple thérapie avec la warfarine chez des patients en fibrillation auriculaire (FA) qui ont subi une PCI avec stent (REDUAL-PCI)
Objectif
L’objectif principal de cette étude est de comparer une double thérapie antithrombotique (DTA) à une triple thérapie antithrombotique (TTA) chez des patients en FA qui ont subi une intervention coronarienne percutanée avec stent (élective ou pour syndrome coronarien aigu). La DTA associe du clopidogrel ou du ticagrélor au dabigatran étexilate à la dose de 110 mg x 2/j (110 mg DE-DTA) ou de 150 mg x 2/j (150 mg DE-DTA). La TTA associe la warfarine, le clopidogrel ou le ticagrélor et l’AAS à une dose ≤ 100 mg x 1/j (warfarine-TTA).
L’étude vise à montrer la non-infériorité de chacune des doses de DE-DTA par rapport à la warfarine-TTA en ce qui concerne la sécurité. Cette dernière sera déterminée par comparaison des taux d’évènements hémorragiques, évalués à l’aide de la classification modifiée des évènements hémorragiques majeurs et des évènements hémorragiques non majeurs cliniquement pertinents de la Société internationale de thrombose et d’hémostase.
Éligibilité
Critères d’inclusion :
• Patients présentant une FA non valvulaire
• Patient présentant :
Un SCA (STEMI, Non STEMI [NSTEMI] ou angor instable [AI]) traité avec succès par PCI avec stent (stent en métal nu [SMN] ou stent à élution médicamenteuse [SEM]) ou une coronaropathie stable, avec au moins une lésion éligible à la PCI, qui a été traitée avec succès par une PCI élective avec pose de stent (SMN ou SEM)
• Le patient doit pouvoir donner son consentement éclairé conformément aux lignes directrices de la Conférence internationale sur l’harmonisation des bonnes pratiques cliniques et à la législation ou à la règlementation locale.
Critères d’exclusion :
• Patients porteurs d’une prothèse de valve cardiaque mécanique ou biologique
• Choc cardiogénique pendant l’hospitalisation en cours
• Accident vasculaire cérébral dans le mois précédant la visite de sélection
• Patients qui ont subi une chirurgie lourde dans le mois précédant la sélection
